Documents and Policies

All full-time Drew University students are eligible to participate in Campus Recreation’s Club Sport programs.

Please read this document for all Club Sport policies and procedures: Club Sport Manual.

All participants of the Drew Rangerettes Dance Team and the Men and Women’s Rugby teams must complete the following forms prior to participation in practice or competition. This paper work must be completed for each academic year. Once completed, the forms need to be handed to the Director of Campus Recreation in the Simon Forum.

  1. Complete the Drew ~ Club Sport Waiver including a list of 2 emergency contact people (do not list teammates).
  2. Please read and complete the Concussion Agreement form. Call Health Services x3414 to set up a “club sport appointment”.  It’s not an actual appointment, but it will send you an electronic form to complete via myHealth Portal through Treehouse.
  3. After submitting the waiver, use Customer ID code 242 31 BE 597 to complete the 25-minute baseline concussion test which is online at the Impact Concussion Test. This test should be completed within 3 days of your first participation with the team. Make sure to allow pop-ups from the site in order to complete the test. This is a fun test of memory and reaction time.
  4. Finally, complete this form: Club Sport Player Info and return to the Director of Campus Recreation in the Simon Forum.
