Please fill in this form completely. This form MUST be completed by any student who is hosting a guest in the Residence Halls, whether or not they spend the night. A form MUST be on file for your guest to stay overnight, by 5PM the day prior to their arrival.

If you wish to have a guest–you are responsible for all of their actions on campus. You must stay with your guest throughout their entire stay.
