College of Liberal Arts Re-Entry Application
—This form is to be used for students returning within one year of leave.**—
If you have been on a leave of absence (voluntary or involuntary) you must complete this electronic form at least one month prior to the date of return.
Please review the following checklist of items to consider upon return:
- Have all your bills from prior semesters been paid? If not, contact the business office to arrange payment before returning.
- Will you have be applying for financial aid? If yes, please complete the appropriate financial aid forms by the necessary deadlines. Contact the Office of Financial Assistance directly for more information.
- If you have taken courses at another school, did you request that a copy of the transcript to be sent to the Drew Registrar?
- If you were on required leave, you must be approved by the Committee on Academic Standing. Your re-entry application will be forwarded to the Committee by the Registrar.
