Required Health Forms
Drew University is committed to providing a healthful environment for all students and is compliant with all state and federal regulations that apply to the health and safety of students (NJAC 8:57-6.1 to 8:57-6.18). In order to protect students from vaccine-preventable diseases and to comply with New Jersey law and University requirements students must submit the necessary health forms to the Health Service by dates listed below. It is the responsibility of all matriculated students to comply with these requirements, failure to do so will result in registration termination and/or campus housing holds. All documents must be in English.

D.Min Students: August 15
Spring Semester: January 15
EOS Summer: June 15
MAT: May 15
Health forms are mailed to incoming students in the enrollment packet sent by the Admissions Office of the College of Liberal Arts (CLA), Casperson School of Graduate Studies, or the Theological School. If you have not received your health form packet you can print it from this link.
All matriculated full-time and part-time students who attend class on campus at any time are required to provide the following:
- Completed Student Information Form
- Health History Form to be completed by the student
- Physical Examination within one year of starting class required for all CLA students
- Proof of two Measles vaccines after age one and thirty days apart, or laboratory proof of immunity if born after 1956
- Proof of one Rubella vaccination after age one or laboratory proof of immunity if born after 1956
- Proof of one Mumps vaccination after age one or laboratory proof of immunity if born after 1956
- Proof of three Hepatitis B vaccines if enrolled in 12 or more credits
- Proof of current Meningococcal Meningitis A,C,Y, & W-135 vaccination if planning to live in campus housing
- Completion of the Information on Meningococcal Disease and Vaccination Form (page 4 of health form packet)
- Completion of the Tuberculosis Risk Questionnaire (page 4 of health form packet)
- Completion of a negative Tuberculosis (PPD) Skin Test within 6 months of the start of class if you are at risk
Short-Term International Students
Matriculated international students who are enrolled in a short-term program must be compliant prior to their arrival on campus. If they are not compliant when they arrive, they may not attend classes nor may they reside on campus. It is the responsibility of the Admissions Office of the school in the programs of which these students are participating to instruct students about this policy, to provide Health Services with the required forms, and to alert noncompliant students in a timely way.
Non-matriculated Students
College Specials, College Visiting Students, and High School Students are required to comply with all requirements of the health form packet except the physical examination is not required. Non-matriculated students in the Graduate and Theological School Students are required to submit health forms. Community Auditors, Graduate or Theological School Special Students who are not pursuing a degree and are not living on campus are NOT required to submit health forms.
Leave of Absence
Students who take a leave-of-absence longer than one year are required to update their medical records. When a student falls into a high risk group, or has traveled or lived in a high risk country during the leave-of-absence, the returning student is required to have a TB test at the time of their reentry to campus. Students who are continuously enrolled are not required to resubmit their health forms.
Failure to Comply
The Health Service is responsible for maintaining a current list of noncompliant students on the University database. If the required forms are not complete or are not submitted to the Health Service by Friday of the second week of classes the Dean of Campus Life and Student Affairs will be so informed. Students will not be permitted to register for subsequent classes.
Exemptions
The State of New Jersey permits exemption from certain immunization requirements under the following conditions:
Age Exemption: Students born before January 1, 1957 are automatically exempt from the Measles, Mumps, and Rubella vaccination requirement. However, age does not exempt a student taking more than 12 credits from the Hepatitis B vaccine requirement or a new student residing in campus housing from the Meningococcal A, C, Y, & W-135 vaccination requirement and you are still required to complete all other documents in the health forms packet.
Medical Exemption: If the specific vaccine medically contraindicated is Measles, Mumps, Rubella, or
Hepatitis B, a written statement signed by the student’s health care provider must be provided to the Health Service office indicating the following:
- The type of vaccine in question.
- The specific period of time that it is contraindicated.
- The reason for the medical contraindication. The reason given must be based upon a valid medical contraindication as enumerated by the most recent recommendations of the Advisory Committee on Immunization Practices (ACIP) of the United States Public Health Service.
If the specific vaccine medically contraindicated is Measles, Mumps, or Rubella, the student must still obtain and provide the Health Service with the laboratory report of immunity status to that disease. If the student is found through laboratory testing to be susceptible to the disease, application for vaccine exemption can then be reviewed by the Health Service. If the specific vaccination is Meningococcal A, C, Y, or W-135 Meningiti an immunity test is not required. You are still required to complete all other documents in the health forms packet.
Religious Exemption: A student will be exempted from required vaccinations if the student submits a signed written statement to the Health Service explaining how the administration of the particular vaccine conflicts with BONA FIDE religious tenets/beliefs. Exemptions are not given for philosophical or moral objections to immunization. Students granted religious exemptions are expected to provide the Health Service with a record of any previously administered immunizations.
If the specific vaccine medically contraindicated is Measles, Mumps, Rubella, or
Hepatitis B, the student must still obtain and provide the Health Service with the laboratory report of immunity status to that disease. If the student is found through laboratory testing to be susceptible to the disease, application for vaccine exemption can then be reviewed by the Health Service. If the specific vaccination is Meningococcal A, C, Y, or W-135 Meningitis an immunity test is not required. You are still required to complete all other documents in the health forms packet.

Confidentiality
Medical records are confidential and are used exclusively by the Health Service to provide safe and personalized care. The immunization record is an exception and is not confidential, since it must be available to state inspectors and University officers on a need-to-know basis. All medical records are kept on file at the Health Service for nine years after a student leaves.
