Repair Form for Students

Use this form if you are a student and would like to report problems with your phone, or cable TV service, or if your ethernet/internet connection does not work (If your ethernet/internet connection works but you are having trouble with certain network or internet services, visit the CNS support website).

When you submit this form, an e-mail with the information will automatically be sent to the Telecom e-mail address. A telecom employee will review your request and create a work order. You will then receive an e-mail with your work order number, which you can use to track the progress of your work request at the Technology Support Portal. Our objective is to complete 95% of all work requests submitted via this form within 8 business hours.
