New Hire Form

The purpose of this form is to gather information about new employees so that their telecom services can be ready for their start date. This form should be filled out by a new hire's immediate supervisor or whomever is in charge of relating required information to Telecom. When you submit this form, an e-mail with the information will automatically be sent to the Telecom e-mail address.

If the new hire's location is already equipped with phone and data connections, allow 2 business days for all work to be completed. If the location is not already equipped with these connections, more time will be required.

GL Number:
In order to assign an Authorization Code to an employee, a General Ledger number (sometimes called a "Departmental Code") is needed. Most GL numbers used for phone expenses end in 61500. We will NOT assign an Auth Code to ANYONE without a GL number. If you do not know your department's GL number, please contact Diane Zsombik at extension 3328.
