Repair Form for Faculty and Staff
Use this form only if you are a faculty or staff person and would like to report services that have stopped working, such as your phone, or ethernet/internet connection (If your ethernet/internet connection works but you are having trouble with certain network or internet services, visit the CNS support website). DO NOT use this form to request moves, adds or changes.
When you submit this form, an email with the information will automatically be sent to the Telecom email address. A telecom employee will review your request and create a work order. You will then receive an email with your work order number, which you can use to track the progress of your work request at the Technology Support Portal. Our objective is to complete 95% of all repair requests submitted via this form within 8 business hours.
****If a phone or jack ceases to function due to improper use (e.g. the equipment was dropped or spilled on) your department will be charged back for the cost of repairs.****
